We encourage you to obtain and fill out a matching gift form from your employer if your employer offers this benefit.  The Institute cannot request the form on your behalf (most employers do not allow this), but we will gladly help you with any information needed to complete the form.  In most cases, after you fill out the form, it will need to be forwarded to the Institute to verify that we indeed received the gift from you. We will then send it on to the appropriate address for the match.
