As part of our strategic plan, one of our longstanding goals is to continually monitor and improve our programs. To help us accomplish this goal, we need your help.

We are conducting a parent/caregiver survey to help us understand what we are doing well and where we need to improve. If you have received a text and/or e-mail communication in regards to this survey, we invite you to provide your feedback related to the quality of care and your satisfaction with our programs at Siskin Children Institute. All responses will be anonymous, confidential, and will not impact your ongoing care.
